1. Purpose

This SOP sets up the governance of [NETWORK NAME] when this SOP library runs as a network. It describes the Network Office and its governance body, how the network chooses its operating variant and adopts Appendix B Central and Site Responsibility Matrix, how network SOPs, local addenda and site work instructions rank against each other, how network SOPs are approved, versioned and trained on across member sites, and how network management review works. It also states how each Principal Investigator keeps the investigator duties the regulations place on them.

2. Scope

This SOP applies only when the site has adopted Model N (SOP-100 Writing, Approving and Controlling SOPs). A Model S site ignores it.

In scope:

  • the Network Office, the Network Quality Committee and any Network Medical Director
  • the choice between N-Integrated and N-Affiliated and the adoption of Appendix B (F-900-02)
  • the order of precedence between network SOPs, local addenda (F-900-01) and site work instructions
  • approval, version control, training and effective dates for network SOPs at each member site
  • terms the network or affiliation agreement must contain in N-Affiliated
  • network management review

Out of scope: onboarding and exit of member sites (SOP-901 Member Site Onboarding, Integration and Exit), study allocation and central contracting (SOP-902 Network Study Feasibility, Allocation and Central Contracting), site metrics, central QA and escalation (SOP-903 Network Oversight of Member Sites (Metrics, Central QA, Escalation)), shared staff (SOP-904 Shared Staff, Cross-Site Coverage and Central Services), and data, systems and records across sites (SOP-905 Network Data, Systems, Privacy and Records Across Sites). The mechanics of writing and controlling a single SOP stay in SOP-100; this SOP adds the network layer on top.

Where the protocol, sponsor or IRB requirement is stricter than this SOP, the network and the member site follow the stricter requirement.

3. Responsibilities

RoleResponsibilities
Network Governing BodyBoard of directors of the network entity (N-Integrated) or the joint governance board set up by the network agreement (N-Affiliated). Appoints the Network Executive, receives integrity concerns about network leadership through [NETWORK GOVERNING BODY CONTACT] (SOP-704 Research Misconduct and Fraud Reporting), and receives the network management review summary.
Network ExecutiveSenior officer of the network entity (N-Integrated) or the person the network agreement names to lead the Network Office (N-Affiliated). Charters the Network Quality Committee, approves the operating variant and F-900-02, provides resources for the network quality system, chairs network management review, and declares network-wide business continuity events.
Network Quality LeadOwns the network SOP system: the network SOP Master Index, network SOP approval and release, local addendum review, network training assignments and network management review. Acts as secretary to the Network Quality Committee.
Network Quality CommitteeApproves network SOPs and major revisions, approves F-900-02 and changes to Appendix B, approves the network audit plan, decides addendum disputes, and conducts network management review.
Network Medical DirectorOptional. Gives physician review of network SOPs that affect participant safety, consent, IP or investigator duties, and advises PIs. Never replaces any PI's investigator duties.
Network Regulatory Lead, Network Contracts and Finance Lead, Network Clinical Operations Lead, Network Training LeadReview network SOPs in their areas and carry out the central activities Appendix B assigns to them.
Site DirectorAccountable for SOP adoption, addenda, training and compliance at the member site. Signs the site adoption record and each local addendum.
Site Quality LeadWhere Appendix B assigns the role to the site: maintains site work instructions and site training records, drafts local addenda, and reports SOP deviations to the Network Quality Lead. Where the role is performed centrally, the Network Quality Lead does these tasks for the site.
Principal Investigator (PI)Keeps all investigator duties for the PI's studies. Approves local addenda that affect investigator duties, participant safety, consent, IP or data integrity at the PI's site. Raises any network SOP that would prevent the PI from meeting those duties.
All Research StaffFollow the effective network SOPs, the site's local addenda and site work instructions. Complete training before each effective date.

4. Definitions

TermDefinition
Model NThe operating model in which a Network Office and two or more member sites use this SOP library together (SOP-100).
N-IntegratedModel N variant in which one legal entity owns all member sites and employs their staff. N-Integrated defaults apply only where the member site and the network entity are the same legal entity. A separate subsidiary, or a professional corporation under a management services arrangement, follows the N-Affiliated rows for PHI, direction of staff, contract signature, records custody and payments unless counsel confirms otherwise (SOP-900, 5.2.1).
N-AffiliatedModel N variant in which member sites are separate legal entities bound by a network or affiliation agreement.
Network OfficeThe central organization of the network, collectively.
Network Governing BodyThe body with final authority over the network: the board of directors of the network entity (N-Integrated) or the joint governance board under the network agreement (N-Affiliated).
Member SiteA research site (a location or group of locations under one site leadership) that operates under the network SOPs. In N-Affiliated, each member site is also a separate legal entity.
Network SOPAn SOP of this library approved by the Network Quality Committee for use at all member sites.
Local AddendumA controlled document (F-900-01) that adds site-specific content to one network SOP for one member site, within the limits of section 5.5.
Work Instruction (WI)A controlled, site-written document giving step-level detail for one study, system or location. It must not conflict with a network SOP or local addendum (SOP-100).
Network MinimumA requirement in a network SOP that the Network Quality Committee has marked as one no member site may lower.
Appendix BCentral and Site Responsibility Matrix: assigns each major activity to Network Office (C), Site (S) or Shared (C/S), with a default per variant.
Network AgreementIn N-Affiliated, the written agreement between the network entity and a member site entity that sets the terms in section 5.9.

5. Procedure

5.1 Network Office and governance body

5.1.1
The Network Executive appoints the Network Quality Lead and each other Network Office role the network uses, and records the names on F-102-01 Site Organization Chart and Role Assignment Record (network version). One person may hold more than one Network Office role, within the independence limits in SOP-102 Site Organization, Roles and Staff Responsibilities.
5.1.2
The Network Executive charters the Network Quality Committee in writing. The charter states:
  • voting members (at least the Network Quality Lead, the Network Executive or delegate, the Network Medical Director if appointed, and [DEFAULT: one PI and one Site Director] drawn from member sites)
  • quorum and how decisions are recorded
  • meeting frequency, at least [DEFAULT: quarterly]
  • the decisions it owns under this SOP
5.1.3
The Network Quality Lead keeps the charter, minutes and decision log in [NETWORK QUALITY SYSTEM LOCATION].
5.1.4
The Network Governing Body names a contact for integrity concerns about network leadership, recorded as [NETWORK GOVERNING BODY CONTACT] and published to all staff at every member site. The contact must not be the Network Executive, the Network Quality Lead or anyone who reports to them. Concerns received are handled under SOP-704, and the Network Governing Body appoints an independent investigator where network leadership is implicated.
5.1.5
Decision owners for network-level activities that other SOPs refer to this SOP (network management review and local addenda are in 5.10 and 5.5):
  • network audit plan: prepared by the Network Quality Lead under SOP-903 and approved by the Network Quality Committee before the start of each audit year
  • network-wide business continuity event (for example an outage of a shared system or central service affecting more than one member site): declared by the Network Executive, or by the Network Quality Lead when the Network Executive cannot be reached; each Site Director then acts for their own site under SOP-705 Business Continuity and Emergency Preparedness
  • network SOP changes: any staff member sends the request to their Site Quality Lead or directly to the Network Quality Lead, who logs it on F-100-03 (network version); approval follows 5.6
5.1.6
A Network Medical Director, where appointed, advises and reviews. The Network Medical Director does not sign Form FDA 1572 or an investigator agreement for a study unless also named as that study's PI or Sub-I, and does not make eligibility, causality or medical care decisions for another PI's participants.

5.2 Choosing the variant and adopting Appendix B

5.2.1
Before any member site adopts network SOPs, the Network Executive and Network Quality Lead record on F-900-02 Network Operating Model and Appendix B Adoption Record:
  • the variant: N-Integrated or N-Affiliated, with the legal basis (common ownership, or the network agreement and its date)
  • the member sites, their legal entities and their locations
  • for each member site, its entity type: the network entity itself, a separate subsidiary, or a professional corporation under a management services arrangement; for each member site that is a separate legal entity, the Appendix B rows that follow the N-Affiliated default (at least PHI, direction of staff, contract signature, records custody and payments) unless counsel confirms otherwise
  • the HIPAA arrangement that applies across sites (single covered entity, affiliated covered entity, organized health care arrangement, or separate covered entities with business associate or data use agreements), confirmed with the Network Privacy Officer under SOP-905. An organized health care arrangement does not by itself permit sharing PHI for research (SOP-905 5.3.5)
  • the Appendix B column adopted for each activity, with each change from the default and the reason
5.2.1.1
Before F-900-02 is approved, [LEGAL COUNSEL] confirms the corporate practice of medicine rules of the state where each member site operates and records the answer in that site's Appendix A State Law Worksheet. These rules vary by state and may require physician services to sit in a professional corporation separate from the network entity. The Network Quality Lead records on F-900-02 the date of counsel's confirmation for each member site.
5.2.2
The Network Quality Lead starts from the Appendix B default for the chosen variant. A change from the default is allowed when the network can show who has the authority and resources to do the activity. In N-Affiliated, an activity may be assigned to the Network Office only where the network agreement gives the Network Office that authority.
5.2.3
The Network Quality Committee approves F-900-02. Each member Site Director signs the record for their site to confirm the site accepts its assignments.
5.2.4
F-900-02 is re-approved when a member site joins or leaves (SOP-901), when the variant changes (for example, an affiliated site is acquired), or when any Appendix B assignment changes. The Network Quality Lead tells affected staff of the change and assigns training under 5.7.

5.3 Order of precedence in Model N

5.3.1
All Research Staff at member sites apply requirements in this order when they overlap:
  • applicable law and regulation, including the law of the state where the member site operates (Appendix A State Law Worksheet)
  • determinations and policies of the reviewing IRB for the study
  • the IRB-approved protocol and sponsor written instructions that apply to the study
  • network SOPs (this library as adopted network-wide), and site-only SOPs for activities the network library does not cover (SOP-100 5.11.7)
  • the member site's local addenda (F-900-01)
  • the member site's work instructions and job aids
5.3.2
SOP-100 5.1.2 to 5.1.4 apply unchanged, including the rule that nothing overrides an action needed to eliminate an immediate hazard to a participant.
5.3.3
A local addendum takes effect only for the member site named on it and only for the network SOP it amends.

5.4 Network SOP content

5.4.1
Network SOPs are the core library SOPs and the 900 series, adopted network-wide. One version of each network SOP is effective across the network at a time.
5.4.2
The Network Quality Lead replaces the network placeholders ([NETWORK NAME], network systems, Network Office contacts) in each network SOP and leaves site-specific placeholders ([SITE NAME], [STATE], local contacts) to be resolved in each site's adoption record or local addendum.
5.4.3
The Network Quality Committee marks the network minimums in each network SOP. Network minimums include, at least, every requirement that comes from regulation and every [DEFAULT: ...] value the committee sets as network-wide. Network minimums are listed on the network SOP Master Index (F-100-03, network version).
5.4.4
Each Site decision box in a network SOP is resolved either once for the network (recorded by the Network Quality Lead) or per site (recorded in the site's local addendum). The Network Quality Lead records which applies for each box on F-900-02.

5.5 Local addenda

5.5.1
A member site needs a local addendum when it must add content to a network SOP that the network SOP does not hold. A local addendum may:
  • add steps that are stricter than the network SOP
  • add steps required by state or local law, or by the site's IRB or institution (for example a state consent or LAR rule from Appendix A)
  • name local contacts, locations, hours and escalation routes
  • name local systems, equipment and vendors and the steps specific to them
  • resolve Site decision boxes that F-900-02 assigns to the site
5.5.2
A local addendum must not:
  • lower or remove any requirement of law, regulation, the IRB or the protocol
  • remove a step, lower a requirement or lengthen a timeline in the network SOP, including a network minimum
  • change a responsibility that F-900-02 assigns to the Network Office, unless F-900-02 is changed first under 5.2.4
  • move any investigator duty away from the PI
  • create a new form code (site forms are attached to the addendum and referenced by title)
5.5.3
The Site Quality Lead, or the Network Quality Lead where the role is central, drafts the addendum on F-900-01 Local Addendum to a Network SOP. The draft names the network SOP and version, the sections affected, the added text and the reason (law, IRB, local practice, system).
5.5.4
The Network Quality Lead reviews each draft against 5.5.2 before site approval and records the result on F-900-01. A draft that would lower a requirement is returned. Where the site and the Network Quality Lead disagree, the Network Quality Committee decides and records the decision.
5.5.5
Approval signatures for an addendum are those in SOP-100 5.11.3, including the site PI where the addendum affects investigator duties, participant safety, consent, IP or data integrity.
5.5.6
A local addendum carries its own version number (for example, SOP-301-A-[SITE CODE] v1.0) and references the network SOP version it amends. When the network SOP is revised, the Site Quality Lead checks each linked addendum before the network effective date and revises, re-confirms or retires it. An addendum not re-confirmed by the network effective date is suspended until reviewed, and the network SOP alone applies.
5.5.7
Site work instructions follow SOP-100 5.3 and must not conflict with a network SOP or local addendum. The Site Quality Lead keeps them in the site's controlled location and lists them on the site index.

5.6 Approval and version control of network SOPs

5.6.1
The Network Quality Committee approves each network SOP and major revision by recorded vote, after review by the Network Office lead for the subject area. Signatures are those in SOP-100 5.11.3, recorded on F-100-01 SOP Review and Approval Record (network version).
5.6.2
Site-level adoption follows SOP-100 5.11.3 and 5.11.4. The site does not re-approve the content. In N-Integrated, the site signature records implementation; in N-Affiliated, it is the member entity's adoption under the network agreement, and the site's existing procedure stays effective until it is signed.
5.6.3
The Network Quality Lead circulates drafts to all member sites for comment for at least [DEFAULT: 14 calendar days] before approval of a major revision, and records how each comment was resolved.
5.6.4
The Network Quality Lead holds the only editable master of each network SOP in [NETWORK SOP REPOSITORY] and publishes effective versions read-only to all member sites. Local addenda and work instructions are stored with a link to the network SOP version they support.
5.6.5
Minor changes, periodic review and obsolete versions follow SOP-100 5.2.4, 5.7 and 5.8. Deviations from network SOPs follow SOP-100 5.11.6; the Network Quality Lead trends them across sites (SOP-903).
5.6.5.1
A minor revision of a network SOP (one that changes no requirement, responsibility, timeline or network minimum, as defined in SOP-100 5.2.4) is approved by the Network Quality Lead alone, without a Network Quality Committee vote or the comment period in 5.6.3. The Network Quality Lead records the approval on F-100-01 (network version) and lists the minor revisions in the report to the next Network Quality Committee meeting.

5.7 Training and effective dates across sites

5.7.1
The Network Quality Lead sets one network effective date for each network SOP version. The effective date is at least [DEFAULT: 30 calendar days] after approval for a major revision, to allow addendum review and training at every site.
5.7.2
The Network Quality Lead, or the Network Training Lead where appointed, assigns training by role across all member sites and tells each Site Director what is due. Each staff member completes F-100-02 SOP Training Acknowledgment, or the training system equivalent, before the effective date. Training on local addenda is done at the site and recorded the same way.
5.7.3
Before the effective date, each Site Director confirms in writing to the Network Quality Lead that training is complete and addenda are current, or requests a site-specific later date with the reason. The Network Quality Lead may approve a later date for one site for up to [DEFAULT: 30 calendar days]. Until the later date, that site keeps the previous version, and the Network Quality Lead records the site effective date on F-100-03 (SOP-100 5.11.5).
5.7.4
A change required by a new regulation, an inspection finding or a participant safety issue may take effect sooner at all sites when the Network Quality Committee approves an urgent release. The Network Quality Lead documents the reason and the interim training method (for example a read-and-acknowledge notice with a briefing).
5.7.5
SOP-100 5.5.4 applies to untrained staff. The Site Director restricts tasks locally; the Network Quality Lead checks completion across sites.

5.8 Investigator duties stay with each PI

5.8.1
Each PI keeps the general responsibilities of an investigator for the PI's studies at the PI's site (21 CFR 312.60; 21 CFR 812.100; ICH E6(R3) Annex 1, 2.3). Network governance, network SOPs, Appendix B assignments and central services do not transfer those duties.
5.8.2
When the Network Office performs an activity for a PI's study (for example, an IRB submission or a contract), the activity is a service the PI relies on. The PI keeps oversight of it in proportion to its importance to participant safety and data reliability (ICH E6(R3) Annex 1, 2.3.1). The PI may ask the Network Office for the records of any central activity done for the PI's studies, and the Network Office provides them.
5.8.3
A PI who believes a network SOP, central decision or Network Office instruction would prevent the PI from protecting participants or meeting investigator duties raises it with the Site Director and the Network Quality Lead. The PI's decision on participant safety stands while the Network Quality Committee reviews it. The issue and outcome are recorded in the committee minutes and, where study-specific, on F-306-01 in the ISF.
5.8.4
The Network Office must not pressure a PI on medical judgment, eligibility, causality assessment, safety reporting or consent. Reports of such pressure are handled under SOP-704 Research Misconduct and Fraud Reporting.

5.9 N-Affiliated: what the network agreement must state

5.9.1
Before a member site in N-Affiliated adopts network SOPs, the Network Executive confirms that the network agreement, reviewed by counsel, states at least:
  • which network SOPs, network minimums and Appendix B assignments are binding on the member entity, and how changes become binding (for example, by site adoption under 5.6.2, or automatically after a notice period)
  • the Network Office's audit and access rights at the member site, including access to source, ISF, systems and staff for QA under SOP-903, and the member's duty to cooperate
  • data rights: what data and records each party may access, use and keep, the legal basis for any PHI flow (BAA, DUA or participant authorization), and who owns study records and is custodian (SOP-905, SOP-402 Record Retention, Archiving and Destruction)
  • which party signs CTAs and budgets and how funds flow (SOP-902)
  • what happens on conflict between a network SOP and the member's own policies or legal duties: the member entity's legal duties and the investigator's regulatory duties prevail; the member notifies the Network Office in writing within [DEFAULT: 10 business days] of finding the conflict; the parties resolve it by a local addendum, a change to F-900-02, or exit
  • direction of staff: the Network Office directs member site employees only to the extent the agreement allows
  • insurance, indemnity, confidentiality and termination terms, and the exit steps in SOP-901
5.9.2
The Network Quality Lead keeps a copy of each network agreement summary (binding SOPs, audit rights, data rights, conflict clause) with F-900-02.
5.9.3
In N-Integrated, for a member site that is the same legal entity as the network entity, the network entity's corporate policies and employment terms provide the authority. No separate agreement is needed for that site, but F-900-02 still records Appendix B assignments. For such a site, the Network Executive confirms the written authority (for example the management services agreement or a subsidiary resolution) covers the points in 5.9.1.

5.10 Network management review

5.10.1
The Network Quality Committee holds a network management review at least [DEFAULT: annually], and also after any FDA inspection at a member site that ends with a Form FDA 483, any critical audit finding, or any finding of possible research misconduct.
5.10.2
Inputs, in addition to those in SOP-700 Internal Quality Control and Quality Assurance Audits 5.8.2 for each site:
  • member site scorecards and escalations (F-903-01, F-903-02)
  • network SOP deviations, local addenda and their trends
  • CAPA trends across sites, and CAPAs that should apply network-wide
  • status of member site onboarding, integration and exit (SOP-901)
  • allocation and contracting performance (SOP-902)
  • shared staff and central services (SOP-904)
  • data, systems and privacy incidents across sites (SOP-905)
  • adequacy of F-900-02 and Appendix B assignments
5.10.3
Outputs are decisions and actions with owners and dates, including network SOP revisions, Appendix B changes, network-wide CAPAs, resource changes and the next network audit plan.
5.10.4
The Network Quality Lead records the review in minutes signed by the Network Executive, sends a summary to the Network Governing Body, and shares the actions relevant to each site with its Site Director. Each member site still holds its own management review under SOP-700, which may be combined with the network review where the Site Director and Network Quality Lead agree.

5.11 Day-one minimum

5.11.1
A network may start operating under this library once the following are in place. The Network Quality Lead records the date each item was completed on F-900-02:
  • F-900-02 Network Operating Model and Appendix B Adoption Record approved, including the entity type of each member site (5.2.1)
  • Appendix B adopted, with the column chosen for each activity (5.2)
  • the HIPAA arrangement across sites confirmed with the Network Privacy Officer (5.2.1 and SOP-905)
  • for each member site, the signed network agreement (N-Affiliated or a separate entity under 5.9.3) or the corporate authority (a site that is the same legal entity as the network entity)
  • for each member site, an integration plan on F-901-02 (SOP-901)
  • a named network QA contact (the Network Quality Lead or a person that role names) published to every member site
5.11.2
Every other requirement of the 900 series is put in place within [DEFAULT: 6 months] of the date in 5.11.1. The Network Quality Lead keeps a dated list of the open items and reports progress at each Network Quality Committee meeting. Member sites follow their adopted SOPs (SOP-100) for any activity whose network procedure is not yet in place.
